Comprehensive Care Planning

Description

Care planning is a way of working with someone rather than for them. It creates a partnerships between the patient and different treatment teams, bringing together what is important to the young person with what is important for them and the people helping them during their hospital stay. Providing this kind of support involves setting clear boundaries and explaining people’s roles and obligations to the patient. This class offers specific strategies and language that foster a collaborative approach to treatment planning across multi-disciplinary teams and different elements in a person’s wellbeing.
